PPO Network Search
Follow the Steps for each PPO Option
1.) Visit https://www.anthem.com/find-care/
2.) Click Basic search as a guest
3.) In the first dropdown menu, select Medical Plan or Network (may also include dental, vison, or pharmacy benefits).
4.) In the second dropdown menu, select the state Georgia where the plan or network is offered. (For employer-sponsored plans, select the state where your employer's plan is contracted in. Most of the time, it's where the headquarters is located).
5.) In the third dropdown menu, select Medical (Employer-Sponsored)
6.) In the last dropdown menu, select National PPO (BlueCard PPO). Click Continue
7.) Enter your location and search criteria or search by provider.
1.) Visit https://www.cigna.com/
2.) Select Find a Doctor
3.) Select Employer or School
4.) Enter your zip code. Choose Doctor by Type, Doctor by Name, or Health Facilities. Enter search parameters or choose from the dropdown menu and click Search.
5.) Select Continue as Guest
6.) Select Continue.
7.) Select, PPO, Choice Fund PPO.
8.) Choose from list of results
1.) Visit www.https://providersearch.multiplan.com/
2.) Highlight PHCS (Logo on front card) and click Select and Search
3.) Enter your search parameters, your zip code, and select Search.
